Human beings have spent an extraordinary amount of time studying adverse conditions during pregnancy. Researchers have examined maternal stress, malnutrition, pollution, infection, violence, medications, substance exposure, poverty, sleep disruption, occupational strain, inadequate healthcare, depression, anxiety, trauma and countless other variables. They then measure what happens to pregnancies, newborns and developing children when those conditions are present.

I want to reverse the experiment and ask a question humanity has never been able to answer completely: What would happen if every reasonably influenceable adverse condition surrounding pregnancy were removed at the same time?

For this thought experiment, imagine two biologically healthy adults who intentionally conceive a child. Their health is excellent, conception occurs normally and fetal development begins under optimal biological conditions. The pregnant woman is then placed in an environment deliberately designed around her physical and psychological wellbeing for the duration of the pregnancy.

She has excellent nutrition and healthcare, restorative sleep whenever her body requires it, complete financial security, stable housing and freedom from occupational pressure. Her environment minimizes harmful exposures and unnecessary physical strain. She experiences physical safety, emotional security, autonomy, supportive relationships and meaningful social connection. She can move, rest, eat and live according to the changing requirements of her pregnancy. Pain and discomfort receive appropriate attention, her concerns receive serious consideration, and everything reasonably necessary for her health, comfort and pregnancy is readily available.

For approximately nine months, humanity treats the person growing its next member as one of the most biologically consequential environments involved in that new human's development. With those conditions established, the scientific question becomes fascinating: What should happen to the developing child?

Start With the Biology

A fetus develops through continuous biological interaction with the pregnant body. The placenta performs an extraordinary regulatory role between mother and fetus, facilitating the exchange of oxygen, nutrients and waste while participating in endocrine, metabolic and immune signaling. Maternal physiology therefore forms an enormous component of the developmental environment in which fetal growth occurs.

Human physiology accommodates ordinary emotional and physiological fluctuations throughout pregnancy. The larger developmental questions concern substantial or chronic conditions capable of altering that environment. Maternal nutrition influences fetal growth and development. Environmental toxins can interfere with developmental processes. Infections can affect pregnancy and fetal outcomes. Chronic or severe stress can influence maternal neuroendocrine and inflammatory systems. Violence during pregnancy is associated with adverse outcomes. Access to prenatal healthcare influences opportunities to prevent, detect and manage complications. Smoking, alcohol and other exposures can produce measurable developmental consequences, while socioeconomic conditions can influence several of these pathways simultaneously.

Taken together, these findings establish an important starting point for the thought experiment: changing conditions surrounding pregnancy can change developmental probabilities for the child. Existing research frequently examines these variables individually or studies combinations that happen naturally within human populations. Researchers can study improved nutrition, reduced toxin exposure, treatment of infection, changes in stress, improved prenatal care or differences between socioeconomic environments. The unexplored question is what happens when as many modifiable variables as possible are intentionally optimized at the same time.

The Utopian Pregnancy Experiment

Imagine a sufficiently large research population in which modifiable prenatal conditions are systematically optimized. Human individuality would remain an expected part of the outcome, while researchers examined whether the optimized developmental environment changed the incidence, severity or expression of physical, neurological and developmental difficulties across the population.

Based on existing biological knowledge, I would predict reductions in pregnancy and neonatal complications associated with preventable exposures, nutritional deficiencies, harmful substances, inadequate healthcare, excessive physiological strain and environmental hazards. I would also predict improvements in population-level measures of fetal growth and neonatal health wherever those measures are currently influenced by modifiable prenatal conditions.

The more fascinating phase would begin after birth. Researchers would follow these children throughout development and examine cognition, emotional regulation, immune function, metabolic health, cardiovascular health, stress responsivity, behavior, learning, sleep, social development and mental health. Those findings could then be compared with populations conceived and raised under ordinary contemporary conditions. Pregnancy creates the biological starting conditions of a human being who may continue developing for another eighty or ninety years, making long-term follow-up essential to understanding the full consequences of the experiment.

My Hypothesis

This article presents an original hypothesis derived from established principles and findings across developmental biology, obstetrics, environmental health, neuroscience and related fields. The comprehensive experiment described here has yet to be conducted, which leaves its ultimate outcome unknown and scientifically fascinating.

My hypothesis is that systematically optimizing the modifiable physical, psychological, environmental and social conditions surrounding biologically healthy women throughout pregnancy would produce a population with a meaningful reduction in some physical, developmental and psychological difficulties currently observed during childhood, with some effects potentially extending into adulthood.

I further hypothesize that the cumulative effect could exceed what researchers might predict by simply adding together estimates from individual pregnancy interventions. Human biological systems continuously interact. Sleep influences endocrine function, stress influences sleep, financial security influences stress, nutrition influences metabolism, relationships influence psychological wellbeing, environmental exposures interact with biological susceptibility, and healthcare affects whether emerging complications are identified and treated. Simultaneously optimizing several conditions could therefore produce developmental effects that remain difficult to detect when researchers examine one variable at a time.

That possibility raises the central question of this entire thought experiment: How many conditions currently treated primarily as problems within individual children are partly downstream consequences of developmental environments humanity has normalized? The answer remains unknown, and discovering it would require studying the optimized condition rather than continually learning only from varying degrees of adversity.

The Environment Around the Pregnant Woman

Modern prenatal guidance places considerable responsibility on individual maternal behavior. Pregnant women receive instructions concerning nutrition, vitamins, alcohol, smoking, exercise, medical appointments and stress management. Those individual behaviors occur inside a much larger environment that can include employment demands, financial conditions, housing, relationships, healthcare access, discrimination, caregiving responsibilities, physical safety and opportunities for adequate sleep and recovery.

If maternal physiological conditions can influence fetal development, every modifiable system capable of substantially influencing those conditions becomes scientifically relevant to pregnancy. Partners, employers, healthcare systems, governments, families, communities and social structures all become potential components of the developmental environment surrounding mother and child.

This expands the concept of prenatal care considerably. The pregnant woman carries the developing child, while the conditions surrounding her can influence the biological environment in which that child develops. Supporting her physical health, psychological wellbeing, autonomy, safety and stability therefore creates favorable conditions for two interconnected biological systems during a uniquely sensitive period of human development.

Women as Humanity's Biological Portals

Human civilization has repeatedly treated women's needs as secondary, optional, inconvenient or subordinate while simultaneously depending upon female biology for the continuation of the species. Examining that contradiction through developmental biology produces an entirely different perspective on the social value placed on women's wellbeing.

I sometimes describe women as humanity's “secret portals.” The term is metaphorical, while the biology underneath it is concrete: every naturally gestated human begins development inside another human body. For approximately nine months, the physical condition of that body and many aspects of the environment surrounding it become intertwined with the developmental conditions of the next human being.

A civilization intentionally attempting to optimize the health and developmental potential of future generations would therefore have a strong biological reason to make pregnancy conditions an extraordinary priority. The health, autonomy, safety, dignity, comfort and wellbeing of the pregnant woman carry value for her as an individual while simultaneously contributing to the developmental environment of another human being.

This perspective changes the research question itself. Instead of primarily measuring how much adversity pregnancy can tolerate before researchers detect measurable consequences, the Utopian Pregnancy Experiment asks something far more expansive: What becomes possible for human development when the pregnant woman is given every reasonable opportunity to thrive?

What I Predict We Would Discover

I predict that the hypothetical population would retain enormous variation in intelligence, personality, temperament, abilities, interests and development because variation is intrinsic to humanity. The more interesting outcome would be whether the population baseline for preventable physical, developmental and psychological difficulties shifted when prenatal conditions were comprehensively optimized.

My hypothesis predicts that researchers would discover that humanity has underestimated the cumulative developmental cost of many conditions routinely placed around pregnant women. A portion of childhood difficulty currently considered part of the ordinary population baseline could decline when modifiable prenatal conditions are optimized simultaneously. The size of that portion is unknown, and determining its magnitude would be one of the primary purposes of the experiment.

Such findings could radically expand the meaning of prenatal care. Housing security, protection from violence, adequate financial resources, nutritious food, appropriate working conditions, restorative sleep, emotional safety, autonomy, supportive relationships and high-quality medical care could all be understood as components of the environment surrounding fetal development. Prevention would begin with the conditions under which a human being is constructed.

The Experiment Humanity Never Ran

The proposed experiment begins with healthy adults, healthy conception and the deliberate optimization of every reasonably modifiable condition surrounding pregnancy. The pregnant woman receives exceptional safety, resources, healthcare, nutrition, autonomy, support, rest and physical wellbeing throughout gestation. Researchers repeat the model across a sufficiently large population and follow the resulting children for decades, measuring physical health, neurological development, cognition, stress responsivity, emotional regulation, behavior and long-term wellbeing.

The results could reveal the actual magnitude of the effect. Small improvements would teach researchers something important about the limits of prenatal environmental optimization, while large improvements could transform assumptions about prevention, pregnancy and developmental health. Either outcome would answer a question that conventional observational research cannot completely resolve because humanity has never established the fully optimized condition as the experimental starting point.

My hypothesis predicts that exceptional care of the people growing the next generation would produce healthier developmental starting conditions for the next generation, with consequences extending beyond immediate birth outcomes. Science already possesses many individual pieces that make this prediction plausible. The unanswered question concerns what happens when those pieces are deliberately assembled into one comprehensive developmental environment.

Imagine asking a pregnant woman, for the entire duration of gestation: What would you require physically, psychologically, socially and environmentally to thrive? Then provide those conditions as completely as humanly possible and study what happens to the developing child.

Humanity has extensively studied what happens when pregnancy occurs under adversity. The Utopian Pregnancy Experiment asks researchers to study the other end of the spectrum with equal seriousness: What happens to human potential when the person creating its first developmental environment is given every reasonable opportunity to thrive?
